Tear trough filler is a hyaluronic acid gel injected into the groove between the lower eyelid and the cheek. It softens a hollow that casts shadow beneath the eye, and it treats under eye hollows, not bags. At The Pinewoods Clinic in Waterloo, Liverpool, treatment takes around 30 minutes and costs £400 per syringe, and one syringe is normally enough.

How does under eye filler work?

The tear trough runs from the inner corner of the eye down towards the cheek. When volume is lost with age, or where the groove is naturally deep, it catches the light unevenly. This leaves a shadow that can look like a dark circle, even when the skin itself is not dark.

Tear trough dermal filler replaces that volume with hyaluronic acid, a substance found naturally in the skin. The gel is soft and fine, made for the thin skin beneath the eye, and the change is visible as soon as treatment is finished.

Results typically last between 9 and 18 months, depending on the product used and how quickly your body breaks down hyaluronic acid, and a top-up can maintain the result. The aim is a smoother join between the lower lid and the cheek, not a change to the shape of your eyes.

Filler or lower blepharoplasty, which do you need?

Under eye filler treats a hollow. Lower blepharoplasty treats a bag. The two can look similar in the mirror, but they have different causes, and filler cannot correct a bag.

A hollow is lost volume, so it is corrected by adding volume back. A permanent bag is fat that has moved forward beneath the lower lid. Placing filler around a bag can make the area look heavier, and lower blepharoplasty is the treatment that removes it. Puffiness that is worse in the morning is fluid, and neither treatment will change it.

Many people have a little of each. Your consultation establishes which is causing the shadow you see, and if surgery would serve you better than filler, you will be told so.

How much does tear trough filler cost?

Tear trough filler at The Pinewoods Clinic costs £400 per syringe, and one syringe is normally enough. Your consultation with Mr Laycock is £180, and there is no consultation fee if you decide to go ahead with treatment on the same day.

Two things can change the final figure. A deeper groove can need more filler than a mild one, and the upper cheek sometimes needs support too, since hollowing beneath the eye can be linked to volume loss just below it. Who is under eye filler suitable for?

Tear trough treatment suits adults whose main concern is a hollow or groove beneath the eye, with little or no bag. The signs are a shadow that concealer covers but cannot lift, a visible line where the lower lid meets the cheek, and a groove that has either deepened over the years or has always been there.

It is less suitable where there is a pronounced bag, a lot of loose skin beneath the eye, or a tendency to swelling in the lower lid and upper cheek, because filler in these areas can hold fluid and look puffy. Dark circles caused by pigment in the skin are not a volume problem, and filler will not change them. Pigmentation is treated with laser instead, which can be used on pigmentation on any area.

Filler is not given to anyone under 18, which is a legal requirement in England.

Fat transfer under the eyes

Fat grafting under the eyes, also called fat transfer, uses your own fat instead of hyaluronic acid. A small amount is taken from another area, such as the abdomen or thigh, then prepared and placed in fine layers beneath the eye. Some of the transferred fat settles in and stays, which is why it lasts longer than filler, while some is reabsorbed over the first few months, so more than one session can be needed.

Your consultation is with Mr Rob Laycock, Maxillofacial and Facial Plastic Surgeon (GMC 6145445), who would carry out your treatment. He assesses the tear trough, lower lid and cheek, confirms whether your concern is a hollow, a bag or pigmentation, and explains what treatment can and cannot achieve for you. The benefits and risks are discussed in full, including the rare but serious risks of injecting near the eye, so you can make an informed decision. There is no obligation to book.

If you decide to go ahead on the same day, treatment can take place at the same appointment and there is no consultation fee.

What happens during your treatment?

The appointment takes around 30 minutes at our Waterloo clinic. Numbing cream is not used, but you are welcome to apply your own before you arrive. Mr Laycock cleans the skin, then places the filler in small amounts, checking the result from different angles as he goes. You see the result in a mirror before you leave, and he talks you through your aftercare instructions.

Tear trough filler aftercare and recovery

Mild swelling and tenderness in the first few days are expected and settle on their own. You will be given written aftercare instructions covering makeup, exercise and alcohol, and who to contact if you have any concerns.

Frequently Asked Questions

Does under eye filler hurt?

Numbing cream is not used at the clinic, but you can apply your own before your appointment if you would like to. Most people describe a feeling of pressure rather than pain, and some tenderness can last for a day or two afterwards.

Does under eye filler cause bruising?

Small bruises at the injection points are common and usually fade within a week. Your clinician will explain how to lower the chance of bruising at your consultation.

How long does under eye filler take to settle?

The change is visible straight away, but the final result is judged once any swelling has gone down, usually after around two weeks. Swelling in the first few days is normal and does not reflect the finished result.

Can under eye filler be dissolved?

Yes. Hyaluronic acid filler can be broken down by an enzyme injected into the area, and Mr Laycock dissolves filler at the clinic, including filler placed elsewhere. It costs from £250 for the initial consultation and first vial, with each additional vial £50.
